We have added a few new features and a few minor bug fixes. Note the red text at the top of the first page, this will allow supervisors to quickly identify if the officer is using the latest form. Officers can now also edit the PersonID field should there be any errors. We also have an error check to make sure there are no duplicate PersonIDs. Right click on the links below and then save the file to your shared drive. Please remove your old out-dated fillable PDF.
Please remember to right click on the file and go to properties, to make it read only so that your blank copy is not over written. Please remember to set your file to “Read Only” once placed on your shared drive!!!
